2. Track : Heart Failure:

Heart Failure is generally referred aCongestive heart failure (CHF). Heart Failure occurs when heart is unable to pump the blood which results lack of blood flow to meet the body’s need. The conditions which leads to heart failure, such as narrowed arteries in heart or high blood pressure, weakens the heart or makes it stiff to fill and pump efficiently.

The main manifestations of Heart Failure include:

  • Shortness of breath
  • Excessive tiredness
  • Chest pain
  • Angina

Types of Heart Failure:

There are many different types of heart failure:

1. Left-sided heart failure:
The most common form of congestive heart failure is Left-sided heart failure. The left side of the heart is responsible for pumping blood to the rest of the body. Blood backs up into the lungs as it is not effectively pumped away from the heart. This can cause shortness of breath and fluid buildup.

Heart failure treatments can improve the signs and symptoms of heart failure and help you live longer. People suffering from Diabetes, High blood pressure, Obesity & Coronary artery disease should control the conditions & prevent heart failure

2. Right-sided heart failure:
Right-side failure is, most often, caused by fluid build-up in the lungs due to left-side failure. The right side of the heart pumps blood to the lungs where it collects oxygen & Sometimes it can occur due to other conditions, including lung disease.

3. Diastolic heart failure:
This occurs when the heart muscle is stiffer than normal. Because the heart is stiff, it does not fill up with blood properly; this is known as diastolic dysfunction. Because the heart does not fill up with blood, it cannot pass as much blood around the body as is necessary. This can occur on either side of the heart

4. Systolic heart failure:
Systolic dysfunction describes the heart's inability to pump efficiently after filling with blood. It often occurs if the heart is weak or enlarged. This can occur on either side of the heart.

Heart Failure can be prevented by reducing the risk factors. For example, by making lifestyle changes along with the help of any needed medications we can also eliminate or control many of the risk factors for heart disease like high blood pressure and coronary artery disease.

3. Track: Valvular Heart Disease:

Valvular heart disease is any disease process involving all the four valves of heart. These conditions occurs on aging. However it may also result for congenital heart defects & also include rheumatic heart disease.

Cardiac skeleton is made up of dense connective tissues which are responsible for the regulation of blood flow through the heart & great vessels. Failure of valve or dysfunction results in diminished heart functionality.

There are two major valvular heart diseases:

  1. Valvular Stenosis: This a condition occurs when the opening of valve is smaller than normal due to fused or harden leaflets. The narrowed opening leads heart to work very hard to pump blood through it which results in heart failure. It also leads to few other symptoms which can effect all four valves can be stenotic ( condition where blood flow restricts & heart valves hardens), the conditions are called as Tricuspid stenosis, pulmonic stenosis, Mitral stenosis or Arotic stenosis
  2. Valvular Insufficiency: This a condition also called regurgitation, incompetence or "leaky valve", this occurs when a valve does not close tightly. If the valves do not seal, some blood will leak backwards across the valve. When the heart leaks continuously, the heart has to work harder to stop the leakage of blood, that results in less blood may flow to the rest of the body. Resulting on which valve is affected, the conditioned is called Tricuspid Regurgitation, Pulmonary Regurgitation, Mitral regurgitation.

    Depending upon the type and severity of valvular disease, the heart dysfuction or Valve failure can result in diminished heart functionality. Anatomically the valves are responsible for the regulation of blood flow through the heart and great vessels, as they are part of the dense connective tissue of the heart known as the cardiac skeleton. Treatment of damaged valves may involve medication alone, but often involves surgical valve repair (valvuloplasty) or replacement (insertion of an artificial heart valve).

4. Track : Congenital Heart Diseases:

Congenital heart defect (CHD) is generally called as congenital heart anomaly or congenital heart disease; it is caused due to problem in the structure or abnormal structure of the Heart at the time of birth. The symptoms are unrecognized until later in life. However the main cause is unknown. It is assumed as certain reasons may be due to infectious disease such as Rubella during pregnancy, use of alcohol or tobacco, Poor nutrition or obesity in the mother. Parent with Congenital heart disease may also be considered as a risk factor.

There are many conditions associated with heart defects including Turner syndrome, Down syndrome, Marfan syndrome. Congenital heart defect are categorized as cyanotic heart defects & non-cyanotic heart defects, depending on the condition whether the child has the potential to turn bluish in colour.

Congenital heart defect may also affect interior walls of the heart, the blood vessels, heart valves, that may lead to heart

To diagnose the congenital heart defects there are special heart tests. It is often diagnosed during pregnancy or soon after birth. The major signs & symptoms of severe defects in newborns include:

  • Fatigue (Tiredness)
  • Rapid breathing
  • Poor blood circulation
  • Cyanosis - a bluish tint to the skin, lips, and fingernails

However congenital heart defects could not be diagnosed until children or older. Children diagnosed with congenital heart defects generally don’t need any treatment. However in severe conditions we need to treat it. The treatment includes medicines, surgery or catheter procedures. It totally depends on the complexity of the defects & also on the child’s age, size

5. Track:  Pediatric Cardiology:

Pediatric Cardiology deals with various heart diseases in children, infants, new bornAtherosclerosis is major cause for Pediatric cardiovascular sickness. Pediatric pericarditis is an irritation of pericardium, the sac that encompasses the heart.

Stroke can even take place in infants, newborns & children. The major reasons for heart stroke in children are different heart diseases, abnormal blood vessels in the brain or due to heart surgery

There are many ways to diagnose or detect heart diseases in childrenPediatric electrophysiology is a process of evaluating the electrical conducting system of heart in children & in infants. Any abnormalities in electrical conducting system in children may result in heart palpitations, arrhythmias, syncope.

There are also different ways of heart surgeries in infants. Pacemaker is a device which can be places in chest or abdomen of child to control the abnormal rhythms of heart using low energy electrical pulses. Heart transplantation in infants is globally accepted therapy. It is done in in children due to congenital malformations. The survey report tells that the survival rate in children after heart transplant is above 20 years & more than 70% recipients survive at least 5 years.

A congenital heart defect in which oxygenated and deoxygenated blood enters the circulatory system resulting from a ventricular septal defect (VSD) right beneath the aorta. This is the condition which causes a bluish-tint, cyanosis, hypoxemia and also has  oxygen deficieny to their tissues in new born.

The constant communication between the heart and the brain has proved invaluable to interdisciplinary fields of neurological and cardiac diseases

The neural rhythms deliver information on static state conditions of healthy individuals. Variations in the neural rhythms provide evidence that a problem is present regarding physiologic regulation and help physicians determine the underlying condition quicker based on the given symptoms.

7. Track: Cardic-Oncology:

 Cardiac oncology is also called as Cardiovascular oncology is a field of medicine by where molecular & clinical alterations in cardiovascular system during the treatment of cancer due to chemo & target therapy. Although many cancer patients also have health conditions related to lung disease, & other infections or kidney disease, the rise of cardiac diseases in these patients has led to the growth of this Cardio-oncology. It  involves cancer doctors (oncologists), cardiologists and researchers. 

Cardiovascular complications that are occurring due to cancer & its treatment have become a major clinical issue which may lead to acute complications during therapy which may limit the effectiveness of cancer treatment.

It is an extremely rare form of cancer that is divided into primary and secondary tumors of the heart. Dyspnea, thoracoabdominal pain, fatigue, hemoptysis, and vomiting, fever, weight loss are usually non-specific symptoms of heart tumour patient. These symptoms make symptoms of other heart diseases, which can make difficulty in diagnosis.

In most cases, the diagnosis is based on clinical history, echocardiography, a CT scan or an MRI scan. Cardiac tumours are often first diagnosed after the patient has had a stroke, an embolism caused by detached tumour tissue.

8. Track: Cardiomyopathies:

Cardiomyopathy is described as a group of diseases that affect the heart muscle. This condition occurs when heart fails to contract properly & the oxygenated blood is not pumped efficiently to the organs of the body which leads to weakness, fatigue, shortness of breath or chest pain.  Cardiomyopathy is categorized as hypertrophic cardiomyopathy, dilated Cardiomyopathy, takotsubo Cardiomyopathy & restrictive Cardiomyopathy. 

In dilated Cardiomyopathy the ventricles enlarges & weakens, in restrictive Cardiomyopathy the ventricles stiffens, in hypertrophic Cardiomyopathy heart muscles enlarges or thickens.

 Cardiomyopathy is also associated with an electrical rhythm disturbance, which may cause heart palpitations (abnormal heart beats). Irregular heart beats leads or skipped heart beats are called as ventricular fibrillation.

9.Track: Hypertension and Venous Thromboembolism:

Hypertension is forced exerted by the blood against walls of the blood vessels. The pressure depends on the work being done by heart & resistance of the blood vessels. The normal normal blood pressure of an individual is 120/80 however blood pressure higher than 130/80 of mercury (mmHg). High blood pressure is common condition in individuals which is usually a common & long-term force & if left untreated can cause heart problems, such as heart diseases. The more the blood your heart pumps & the narrower your arteries, the higher your blood pressure. Blood pressure usually do not have any symptoms for years & can damage to blood vessels & causes heart problems.

Pulmonary hypertension is stated as a condition where the blood pressure is increased within arteries of the lungs.  The manifestations include Syncope, shortness of breath, chest pain, swelling of legs, fast heart beat. However the cause is unknown.

Thromboembolism is generally obstruction or blockage of blood vessels by a blood clot that dislodged from its origin site elsewhere within the cardiovascular system.

10. Track:  Diabetes and Cardiometabolic Disease:

 Diabetes mellitus (DM), is generally a group of metabolic disorders where the blood sugar level rises for a prolonged period of time. The manifestations of high blood sugar include frequent urination, increased thirst & weight gain. If left untreated, diabetes can cause serious issues.

Acute complications include hyperosmolar hyperglycemic state, ketoacidosis or death. When diabetes left untreated can cause Stroke, cardiovascular diseases, foot ulcers, damage to the eyes.

Cardiometabolic compliactions can cause many diseases where a wide spectrum of different factors including lifestyles, diets, living environments, & related genetic factors.  Genetic factors may include hypertension, Diabetics etc. Cardiometabolic compliactions if unrecognised leads to cardiometabolic risk where you are prone to have diabetes, heart diseases & stroke.

      Cardiac Computerized Tomography (CT) Scan:

      A computed tomography (CT) scan of the heart is a technique of imaging heart using x-rays to create detailed pictures of the heart and its blood vessels.

      This test is also called as a coronary calcium scan as it is done to see if you have a build-up of calcium in your arteries.

      It is called CT angiography as it is done to scan the arteries that bring blood to your heart. This test helps to evaluates if there is narrowing or a blockage in those arteries. 

      Cardiac Magnetic Resonance Imaging (MRI):

      Cardiac magnetic resonance imaging (MRI) is used is used to detect & monitor cardiac disease and evaluate heart’s function in patients with congenital heart disease & other cardiovascular diseases. It uses a powerful magnetic field, which creates radio waves and a computer to produce detailed pictures of the structures within the heart

      Chest X ray: An X-ray is a generally a process of imaging test that uses small amounts of radiation to produce pictures of the organs, tissues, and bones of the body. When focused on the chest, it can help spot abnormalities or diseases of the airways, blood vessels, bones, heart, and lungs. Chest X-rays can also determine fluid in patient’s lungs, or fluid or air surrounding patient’s lungs

Cardiac Surgery is usually performed on the heart or blood vessels by a cardiologist or cardiac surgeon. It is used to treat complications in heart disease like congenital heart disease, to treat valvular heart disease, coronary artery bypass grafting, rheumatic heart disease, endocarditis & atherosclerosis. Heart transplant is the major part of cardiac surgery.

There are different heart surgeries performed

·          Repair/replace heart valves, which control blood flow through the heart

·         Repair of abnormal or damaged structures in the heart

·         Implant medical devices that help control the heartbeat or support heart function and blood flow

·         Replace a damaged heart with a healthy heart from a donor

There are many traditional & oldest heart surgery performed which is Open-heart surgery. It is usually performed by opening the chest wall to operate on the heart. The cut is done on the breastbone or on the upper part of it to open the chest. Once the heart is exposed the patient is connected to heart-lung with bypass machine. The machine takes the heart pumping activity & moves blood away from the heart. This allows surgeons to operate on heart which is unable to pump the blood & heart isn’t beating.

There is also a type of heart surgery is called Off-pump, or beating heart, surgery. It's like a traditional open-heart surgery wherein the chest bone is opened to access the heart & the heart isn't stopped, and a heart-lung bypass machine isn't required.

In this procedure small incisions between the ribs to do different types of heart surgeries & breastbone is not opened to reach the heart. This is called Minimally invasive heart surgery. In this procedure heart-lung bypass machine may be used or may not be used.

The main advantage of Newer methods of heart surgery (such as off-pump and minimally invasive) may reduce risks and speed up recovery time. Studies are under way to compare these types of heart surgery with traditional open-heart surgery.
